1. From the menu at the top of your
     browser, please click on the file menu
     and select ‘Page Set Up’.

2. The pop up box will shown on the
     right will appear.

3. From the Orientation option on the
     bottom left hand side of the box,
     select Landscape.

4. Click on the OK tab at the bottom of
     box.

5. From the file menu select the ‘Print...’
     option. To print one copy of the
     form, click on the OK tab at the
     bottom of the box.

6. To return to the original ‘Portrait’
     settings, follow steps 1-4 but change
     the selection in step 3 to ‘Portrait’.
